What we provide
A steady foundation for independent living
At Assisted Care, every home comes with people behind it. We give adults a safe, settled place to live and the everyday support to rebuild confidence, learn new skills and move forward at their own pace.
Personal support from the start
You'll have a dedicated coordinator from day one. We provide personalised support and regular check-ins, with on-site support available whenever you need it.
Homes that feel right
Comfortable, fully furnished homes where you can settle, feel safe and focus on what comes next. All are dry houses, and you'll need to be abstinent from alcohol and non-prescribed drugs for at least 6 months.
Support for your goals
We work with you to build independence, confidence and life skills, so you can achieve your goals and maintain your tenancy.
Eligibility
Who we can support
We provide supported accommodation for adults aged 18 to 65 who are homeless or at risk of homelessness and have identified support needs our service can meet.
We can accept
- Adults aged 18 to 65
- Homeless or at risk of homelessness
- An identified support need we can meet: mental health, substance recovery, physical or learning disability, or resettlement after offending
- Willing to engage with support
- In receipt of benefits
- Abstinent from alcohol and non-prescribed drugs for at least 6 months
- At least 6 months after release, where there is an offending history
- Two valid forms of ID, proof of National Insurance number, and recent proof of benefits
We cannot accept
- Any history, actual or attempted, of sexual offences
- Any history, actual or attempted, of arson
- Any history, actual or attempted, of attacks on persons or property
Exclusions relating to sexual offences, arson, and attacks on persons or property are absolute, with no discretion.

How referrals work
Referrals are accepted from professionals: local authorities, care coordinators, probation officers, and support agencies. Complete the online referral form with the client wherever possible.
If anything is missing, we’ll get in touch and help you complete it.

After submission
The journey after referral
Review within 24 hours
Our team reviews the referral against our eligibility criteria and comes back to you.
Assessment
If the referral is a good fit, we invite the client for an assessment in person to understand their needs and circumstances.
Allocation
We match the placement to the right house, based on the support need, the client, and current availability.
Moving in
A dedicated coordinator welcomes the resident on day one and works with them to shape their support around their goals from the start.
Who we work with
Working alongside local agencies
We accept professional referrals from the people who already know a client. Referrals should be completed with the client wherever possible, and we’ll work with you to make sure we have everything we need. We also work alongside registered providers of social housing.
