Complete the form below to make a professional referral to Assisted Care.
We welcome referrals from local authorities, care coordinators, probation officers, support workers, and other professionals. We respond to every referral within 24 hours.

Self-referrals are not accepted. Referrals should be completed with the client wherever possible.
Before you start
Please check the client meets our eligibility criteria before completing this form. If anything is missing, we’ll get in touch to help you complete it.
- ✓ Adult aged 18 to 65
- ✓ Homeless or at risk of homelessness
- ✓ Identified support need we can meet (mental health, substance recovery, physical or learning disability, resettlement after offending)
- ✓ Currently in receipt of benefits
- ✓ Willing to engage with support
- ✓ Abstinent from alcohol and non-prescribed drugs for at least 6 months
- ✓ Where there is an offending history: at least 6 months after release
- ✓ Two valid forms of ID, proof of NI, and recent proof of benefits available to upload
Absolute exclusions (no discretion): any history, actual or attempted, of sexual offences, arson, or attacks on persons or property.
